Requirements for COSMOS Access:
COSMOS access is available to University of Arizona administration, faculty and staff. In addition to being a University employee, applicants must meet the following requirements before access can be established:
- UA NetID
- Complete the online FERPA Web Course
- This web course reviews the federal law that governs the rights of students and institutional responsibilities with respect to student records
How to Request COSMOS Access:
New Account Access
To request new account access, the employee's Supervisor must complete a Request for Account Access form and send it to the appropriate Account Representative(s) listed on Page 2 of the form.
NOTE: We recommend the use of version 6.0 or greater of Adobe's Acrobat Reader. Download the latest FREE version of Adobe Acrobat Reader.
Additional Access for Existing COSMOS Users
To request modification to an existing account access, the employee's Supervisor must complete a Modification of Account Access form, which includes their original signature (no initials), and send it to the appropriate Account Representative(s), listed on Page 2 of the form.

How to Delete COSMOS Access:
Delete COSMOS Access
To request deletion of an existing COSMOS account, the employee's Supervisor must complete a Request for Account Delete form and send it to the appropriate Account Representative(s) listed on Page 2 of the form..
